SAVE A BUDDY FUND “Pit Stop Program” Spay-Neuter Application for
LAKE COUNTY RESIDENTS
The “Pit Stop Program” is for pits and pit mixes, males and females!

Please fill out this application in full. Once we receive it we will mail you a spay-neuter voucher valued at $75.00 (provided we have funds available). The voucher is to be used to offset some of the cost of your dog’s spay-neuter surgery. Bring your voucher with you to your dog’s spay-neuter appointment. The voucher will be accepted as cash by your veterinarian. Any additional cost above the value of the $75.00 voucher for spaying-neutering your pet is your responsibility. You must receive your voucher from us BEFORE you take your pet in for surgery! There are no income restrictions to participate in this program and you may combine our voucher with any financial assistance you receive from other spay-neuter organizations. Please allow up to 10 days to receive your voucher (schedule your appointment accordingly).
